Discharge Planning For Chronically/Terminally Ill Patients

The lack of establishment of continuum of health care for discharge patients suffering from HIV related illness is a thorn in the flesh. HAQOCI has undertaken to strengthen the discharge process of patients thought training in discharge planning. The chronically ill patients require a comprehensive support system for continuing care in a home setting.

The purpose of the discharge plan is to ensure continuity of quality if care by preparing the family and patient for referral to the relevant level of care or a community home based care programme.

A training module for participants is currently being developed. This module will greatly facilitate standardization of content and methods of training on discharge planning in the future. Also being developed is a hospital discharge plan flow chart.
